Serial # (found on the back of your tablet, which starts with S/N)
Drawing software and its version (please provide all software that has this issue)
Link to a video capturing the shortcuts you set on the driver, then switching to your drawing software and showing that the shortcut didn't work. Please ensure the restriction on the video link is removed.
